Adult Carers Practitioner

£23418 or £23,975 – pro-rata depending on experience and skills

Full-time (37 hours per week) or opportunity for Job share
Part-Time Fixed Term Contract until 30th September 2024 (22.5 hours per week)

Job Description and Person Spec

You will support, inform and upskill adult carers to maintain a healthy life balance through their caring role and to help increase their knowledge and understanding. You will complete person-centred Carers Assessments, Reviews and Support plans for carers who are caring for someone over the age of 18 years. You will help to promote media resources that enable self-help support, including digital inclusion enabling them to take control of their own lives. Assist Carers Trust Solihull to enable support to carers, professionals and other CTS staff with ongoing training, awareness, gaining skill and understanding by Leading in areas of specialist work.

To ensure carers are involved in all aspects of service planning and shaping of services that affect them.

Young Carers Activities Co-ordinator

£23418 or £23,975 – depending on experience and skills

Full-time (37 hours per week) Fixed Term for 12 months with possibility of extension

Job Description and Person Spec

Working as part of a dynamic, fun and dedicated team you will support young and young adult carers aged 5 – 25 to maximise their personal, social, health and emotional wellbeing and achieve equal access to fulfilled lives.

You will plan and organise the delivery of a variety of activities and learning experiences for young carers.  You will ensure respite breaks are accessible to all, improves social and emotional wellbeing, offers peer support and motivates young carers to participate in external community-based activities.  You will also share the lead in organising local large events for Carers Trust Solihull (CTS).

We have opportunities based in various locations across Solihull for carers, in our offices or community based provision such youth clubs or coffee mornings.

We provide Core Training for all of our volunteers when they start their placement with us, so they are already skilled and confident enough to work with the team and carers from day one.
